Assume technical responsibility before the Entidade Reguladora da Saúde (ERS) and Instituto da Segurança Social, I.P. (ISS),
Develop the Individual Care Plan (ICP) for each patient,
Implement care protocols for dementia and post-hospital discharge support,
Conduct initial diagnostic assessments via teleconferencing and maintain close communication with family members.
Requisitos
Higher Education: Mestrado Integrado em Medicina, Licenciatura em Enfermagem, Licenciatura em Psicologia, Licenciatura em Fisioterapia, Licenciatura em Serviço Social (recognized in Portugal).
Professional License: Active registration with the respective professional council (mandatory for ERS licensing).
Experience: Minimum of 2 years in coordination roles in Health Care Services.
Digital Skills: Comfortable with care management software and telecare platforms.
